I grew up all my life in Rome so I might be able to help u.
Not quite sure were u would buy custom made shoes but the city is filled with very good leather shoe shops so I am sure u'll find something.
For vintage Rome isnt' HUGE on vintage u'll find much more stuff in Milan but all the vintage stores are in "via del governo vecchio". It's a very cute street in the center who has cool restaurant and vintage shops. Also very nive jewelery shops. The daughter of fendi owner, delfina delletrez fendi has her shop there. It's her own jewelery line very cool with skulls animals and frogs. One of my all time favorite store is in via del governo vecchio. It's called POEMS it's very cute and frilly and it's half shop/half atelier.
